Responsibilities:
Provide comprehensive nursing assessments with an understanding of the social determinants of health
Complete triage of patients both in person and on the phone to aid in treatment decisions in relation to same day appointments, unscheduled care, and telephone calls from patients
Provide general primary care nursing to patients in keeping with nursing standards
Make appropriate hospital and community referrals and follow up as needed
Participation in education of primary care nursing and physician assistant students
Assist in to ensure smooth and efficient functioning of the clinic
Work collaboratively with other clinic staff and community organizations to improve client care, service delivery, and coordination
Qualifications:
Current Registration with the Ontario College of Nurses in the General Class
BScN preferred
CPR certified- PID clearance required
Ability to effectively triage patients that walk in or call on the telephone
Knowledge of the broader determinants of health, health equity, and patient-centered care
Proven ability to solve problems effectively and independently, organizational skills (managing workflow, assessing and judging priorities, planning, multi-tasking)
Demonstrated ability to work effectively within an interdisciplinary team
Availability to work flexible hours including days
Excellent interpersonal and crisis intervention skills
Experience and comfort working with an electronic charting system is an asset

Experiences:
Preference BScN new graduates may apply
One year clinical experience as a Registered Nurse (in the last three years) in one or more of the following areas: Community Health; Primary Care; Urgent or Emergency Care; and/or Acute or Intensive Care.
Experience with targeted patient populations: patients with chronic diseases particularly diabetes and vascular disease, persons 65 and older, young families, care for patients in the home, patients requiring access to mental health and/or addictions
